Registration

There are three easy ways to register:

1) Register On Line:
Use the form at the bottom of this page - we have a SSL Secure Ceritifcate for this website

2) In person:
Stop by 128 S. 5th Street during our business hours or call us at (970) 255-6757

3) By mail:
Download a class schedule and registration for each student, select the classes you wish to register for, and then send your registration along with payment to (you only need to fill out contact/emergency information once per family):

General Policies:____________________________________________ _______________

1) You may register for classes at any time (based on availability) - tuition will be prorated

2) Regular programs are billed on a monthly basis; 15-day written notice is required for withdrawal from current registration

3) Payment is due by the 5th of each month unless other arrangements have been made; late fees will apply to open balances; partial payments will be allowed if you are registered for the automatic billing option to a credit card

4) Make-up classes are allowed if scheduled in advance and there is availability

5) Most classes have a minimum number of students required

**If you are a returning student, you do not need to fill out all of the contact/emergency information/permission agreement again, unless something has changed....please just list student(s) and their current level and select the appropriate classes for registration**

**If you are registering more than one student, you only need to complete Part II for one of the registrations for emergency contact (if information is applicable to all registrations)**

Please fill out the following form and, if you are new to Creative Avenues, download the
Permission Agreement (page 2) and bring it with you the first day of class (or mail it in prior to classes beginning). We must have this form on file for your student(s) to participate in our program (one form per family).
